Under the direction of the market clinical leadership, with matrix to RCNIO or designate, the Clinical Informaticist is responsible for providing leadership, clinical understanding, & support to the planning, coordination, & implementation and sustainment of clinical health information technology, (HIT). Works collaboratively with, but not limited to, clinicians, physicians, One Care Director, Medical informaticist lead, & IT to ensure that the needs of the patient care areas are met & project timelines, milestones, & deliverables are on target with the overall clinical IT strategy. Provides input regarding the IT system & the impact on existing clinical standards, processes & forms in such a way as to gain support & buy-in from staff to successfully implement and sustain clinical HIT. Position requires an understanding of clinical processes and HIT needs of the clinical disciplines. Will provide guidance of ongoing strategy based on new models of care and optimization of systems, system integration, workflow analysis, analytics, research, outcomes and metric tracking and quality reporting. Demonstrates organizational, & communication skills in dealing with patients, families, physicians, & other health care workers.
As a Registered Nurse, you will be responsible for delivering the highest quality patient care according to the specific orders of each patient’s individual physician. This will involve, utilizing your knowledge and skills to educate patients and their families on prevention and healthy habits.
Additional responsibilities for this health care role include:
Monitoring patients’ conditions and assessing their needs
Collaborating with physicians and nurses to devise individualized care plans for patients
Administering patients’ medications and providing treatments according to physicians’ orders
Fostering a supportive and compassionate environment to care for patients and their families
We are looking for a responsible and compassionate Registered Nurse with top-notch clinical skills, a caring attitude, and the ability to work independently with minimal supervision. It is also important to display excellent verbal and written communication and interpersonal skills as well as the ability to effectively educate patients and their families on the in-home healthcare process.
Additional requirements for this role include:
Current Registered Nurse (RN) license that allows you to practice in our state
Graduate of an accredited nursing program
Five Years Experience as a Registered Nurse preferred
Basic Life Support (BLS) required
Specialty certification (ex: CCRN, CNOR, CEN) preferred
Proficiency in the use of computer applications including EHR required or to be obtained.
Clinical Informatics certification or equivalent desired.
Knowledge and understanding of clinical documentation, informatics, clinical terminology and clinical models required or to be obtained.
Additional certification or licensure may be required for some specialty areas
